Counterfeit Medical Records? A Listener Wants to Know

“Robert,” the TTT listener; Frank Cohen, MPA; and,

“I was an ED patient recently,” wrote Robert, a listener of the last edition of Talk Ten Tuesdays. “After receiving and reviewing my electronic medical record, it (was) revealed (that) the entire physical exam was counterfeit – no exam was performed. I was fully alert and my wife was present, yet (no) complete exam was documented. How will clinical documentation integrity (CDI) address this?”

While this is not a clinical documentation integrity (CDI) issue, but rather one that hinges on risk and compliance, Talk Ten Tuesdays reached out to nationally recognized healthcare attorney and physician John R. Irwin to be the special guest for the milestone 300th edition of the weekly Internet broadcast, which debuted in 2011.

“The ever-increasing pressure on physicians to see more and more patients and at the same time deal with equally increasing administrative and medical record-keeping requirements can sometimes lead to recordkeeping errors or omissions, but also, sadly recordkeeping fabrications,” wrote Irwin, a lawyer for more than 40 years who has also been practicing in the field of medicine for 30 years.
