Are the current evaluation and management (E&M) documentation guidelines outdated and burdensome? Evidently, the Centers for Medicare & Medicaid Services (CMS) thinks so. In fact, CMS intends to undertake a major effort to revise the guidelines, based on provider feedback during the comment period of the 2018 Physician Fee Schedule proposed rule.
